NCHC Youth Scholarship Application

The North Carolina Horse Council will offer a total of 5 scholarships of $100 each to interested middle school or high school youth interested in attending or participating in an equine related activity such as a recognized 4-H event, Pony Club event , summer camp or other equine related program. In addition, scholarship grants may be submitted by a youth to participate in a certified equine therapy program.

To apply, the individual must be of middle school age or older, but not eligible to enroll or enrolled in a post-secondary program. (Exceptions may be made for an individual in a special needs program.)

Application should be typed (12 point font), and include the following:

Personal information

  • Name
  • Address, city, state, zip code
  • Phone number
  • County & District
  • Age & date of birth

High School Information

  • Name of High School attending or attended
  • Cumulative GPA
  • ACT or SAT Scores (if taken)
  • Involvement in clubs or activities at school
  • Awards and recognition at school

4-H, Pony Club, or other equine organization Information

  • Number of years in club
  • Club name (if applicable)
  • Projects held: be sure to include 4H Horse Project Activities etc.
  • Offices or positions held in your club
  • Awards and recognition in club
  • Add community service, volunteer work, involvement with your community, etc.
  • Expand on your involvement with your community, let us know what you do to improve and help your community.

Additional Information

  • Photograph of applicant (no larger than 4×6). Please do not staple photograph to application.
  • Three letters of reference with addresses and phone numbers will also need to be submitted with your application.

NCHC Equine Scholarship Application

The North Carolina Horse Council annually awards two scholarships of $1,000.00 each to North Carolina students who have demonstrated academic success, equine community involvement and volunteerism.

Deadline for application submission is: June 1, 2012

GENERAL INFORMATION

1. All information is to be presented in a folder or binder with a cover sheet including title, applicant name, city, state and date.
2. All information will be held in strictest confidence by the North Carolina Horse Council.
3. All applications and supporting materials become the property of the North Carolina Horse Council and cannot be returned.
4. Upon verification of current enrollment with a university or college in North Carolina in an equine- related major or a North Carolina based horse-related program, a check in the amount of the award will be sent to the appropriate college, university or program for the recipient to be applied to educational expenses. Some examples of courses of studies for which the scholarships are intended are Equine Science/Studies, Equine Business Management, Equine Therapy, Pre-Veterinarian, Farrier Training, Professional Jockey Certificate, Professional Horsemen’s Certificate, Equine Therapist Certificate etc.
5. The North Carolina Horse Council reserves the right to withhold scholarships if there are insufficient numbers of qualified candidates.
6. Incomplete applications will not be considered.
7. Prior scholarship recipients are ineligible for award.
8. Scholarships are awarded only to North Carolina residents attending North Carolina colleges, schools, or programs.
9. All scholarship awards will be made in compliance with the North Carolina Horse Council conflict of interest policy.

□ Typewritten Essay (page 6)
□ Three letters of recommendation (Including recommendation cover letter forms provided with the application, pages 7-9)
□ Official copy of your most recent transcript (high school, college, or proof of GED)
□ Copy of SAT/ACT test results (n/a for technical programs i.e. farrier school)
